Yellow fever is a serious viral infection transmitted by the bite of infected mosquitoes, particularly those active during the day. It is a vaccine-preventable disease and is most commonly found in tropical and subtropical regions of Africa, Central, and South America.

Outbreaks of yellow fever can be severe, with mortality rates reaching up to 50% in some cases. The World Health Organization (WHO) strongly recommends vaccination for anyone travelling to areas where the virus is present. In addition, many countries require proof of yellow fever vaccination for entry, especially if you are arriving from or transiting through an affected region.

To meet these requirements, travellers must obtain an International Certificate of Vaccination or Prophylaxis (ICVP), which is only available through a registered yellow fever vaccination centre.

Yellow fever is a viral haemorrhagic disease transmitted by infected mosquitoes, primarily Aedes aegypti. It occurs in parts of Africa and South America and is named for the jaundice (yellowing of the skin and eyes) that can develop in severe cases. The illness ranges from a mild, flu-like infection to a severe form involving bleeding, organ failure, and death. Because there is no specific antiviral treatment, prevention through vaccination is the most effective defence.

The virus spreads through the bite of infected mosquitoes, particularly in jungle or urban settings. It cannot be transmitted from person to person. Preventing yellow fever involves avoiding mosquito bites by using repellents, wearing long sleeves, and staying in screened or air-conditioned areas. Most importantly, vaccination is essential for those travelling to high-risk areas. Many countries require proof of yellow fever vaccination in the form of an International Certificate of Vaccination (ICVP) for entry or exit.

Symptoms develop 3 to 6 days after infection and may begin with fever, chills, headache, muscle aches (particularly in the back), nausea, and vomiting. While many cases resolve after this initial phase, around 15% progress to a more toxic phase with jaundice, bleeding from the mouth, nose, or eyes, organ failure, and shock. Up to half of those who enter the toxic phase die within days.

The yellow fever vaccine is a live attenuated vaccine given as a single dose, usually at least 10 days before travel. One dose provides lifelong immunity for most individuals. In rare cases, a second dose may be recommended if immunity is thought to be waning or for people at continued high risk.

The vaccine is required or recommended for travel to many countries in sub-Saharan Africa and tropical South America. Additionally, some countries require proof of vaccination if you’re arriving from a yellow fever-endemic area. Without the ICVP (the “yellow card”), you may be denied entry or quarantined.

Most side effects are mild and include headache, muscle pain, fever, or redness and soreness at the injection site. Very rarely, serious side effects can occur, such as allergic reactions or yellow fever vaccine-associated neurological disease (YEL-AND) or viscerotropic disease (YEL-AVD), especially in people over 60 or those with weakened immune systems. Pre-vaccination assessment is crucial.
