Rabies is a viral disease that affects the central nervous system, ultimately causing brain inflammation and death once symptoms appear. It is one of the most feared infectious diseases due to its near-100% fatality rate once clinical signs develop. Rabies is caused by the rabies virus, a member of the Lyssavirus genus, and it infects mammals, including humans. The virus is carried in the saliva of infected animals and can be transmitted to humans through bites, scratches, or contact with mucous membranes.

Rabies is most commonly spread through the bite of a rabid animal—typically dogs in many parts of the world. However, other animals such as bats, foxes, raccoons, monkeys, and cats can also carry the virus. The disease is not spread through intact skin or by casual contact. Prevention relies heavily on vaccination, both before and after potential exposure. Pre-exposure vaccination is advised for individuals travelling to regions where rabies is widespread and where medical care may be delayed or limited. If bitten or scratched, prompt post-exposure prophylaxis (PEP) is essential and can be life-saving.

Initial symptoms can be flu-like, such as fever, headache, and fatigue. As the virus spreads to the brain, it can cause anxiety, confusion, hydrophobia (fear of water), hallucinations, paralysis, and ultimately death. The disease progresses quickly once symptoms appear, and treatment is only effective if given before symptoms develop.

For pre-exposure vaccination, the primary course typically consists of two doses given on days 0 and 7. A booster may be required after one year and then every 3–5 years for continued protection in high-risk individuals. For travellers, immunity may last for several years, but booster recommendations vary depending on exposure risk and vaccine type. After potential exposure, even vaccinated individuals will still require two additional doses.

Rabies vaccination is recommended for travel to areas where rabies is common and where access to post-exposure treatment may be limited or delayed. This includes parts of Asia (notably India, Thailand, and the Philippines), Africa, Central and South America, and Eastern Europe. It is especially recommended for long stays, remote travel, or those likely to work with animals.

The rabies vaccine is generally well tolerated. You may experience mild redness, swelling, or pain at the injection site, and some people report fatigue or a low fever. Serious side effects are very rare.
