Typhoid

Typhoid fever is a bacterial infection caused by Salmonella enterica serotype Typhi, typically contracted through consuming contaminated food or water. A related illness, paratyphoid fever, is caused by Salmonella Paratyphi and presents with similar symptoms, though it is usually less severe.

While the majority of typhoid cases occur in Asia, infections are also reported in other parts of the world, particularly in Africa and regions of Central and South America. Travellers to these areas are at increased risk and should consider vaccination and careful food and water hygiene.

Typhoid fever is a bacterial infection caused by Salmonella enterica serotype Typhi. It affects the intestinal tract and bloodstream, leading to serious illness if left untreated. Typhoid is a major public health issue in regions with poor sanitation and limited access to clean water. While rare in the UK, it is a significant risk for travellers visiting parts of Asia, Africa, and Latin America. Without prompt antibiotic treatment, typhoid can lead to severe complications such as intestinal perforation, internal bleeding, and death.

Typhoid spreads through ingestion of food or water contaminated with the faeces of an infected person. The risk is highest in areas with poor hygiene, especially where food handling and sanitation are not properly managed. Preventative measures include drinking only bottled or properly treated water, avoiding raw or undercooked food, and washing hands frequently. Vaccination is strongly advised for travellers to endemic regions. Two types of vaccines are available in the UK: an injectable inactivated vaccine and an oral live vaccine (not suitable for all individuals).

Symptoms usually appear 6 to 30 days after exposure and include high, prolonged fever, weakness, abdominal pain, constipation or diarrhoea, loss of appetite, and a rose-coloured rash on the trunk. In more serious cases, confusion, delirium, and complications such as bowel perforation may occur.

The injectable vaccine (Typhim Vi) provides protection for around 3 years, while the oral vaccine (Vivotif) offers protection for up to 5 years. Boosters are recommended for ongoing travel to high-risk regions.

Typhoid vaccination is advised for travel to South Asia (especially India, Pakistan, Bangladesh), parts of Africa, Southeast Asia, and Central and South America—regions where typhoid is endemic and sanitation infrastructure may be limited.

Most side effects are mild and include pain or redness at the injection site, low-grade fever, and general tiredness. The oral vaccine may cause mild stomach upset. Severe side effects are rare.
