Japanese Encephalitis (JE) is a viral brain infection transmitted by mosquitoes, primarily found in rural and agricultural parts of Asia and the western Pacific. It is caused by the Japanese encephalitis virus, a flavivirus related to dengue and West Nile virus. While most infections are asymptomatic, a small proportion can lead to severe neurological illness with high fatality rates and long-term disability.


JE is spread through the bite of infected Culex mosquitoes, which breed in rice paddies and stagnant water. The virus circulates between mosquitoes, pigs, and wading birds. It does not spread from person to person. Prevention includes mosquito bite avoidance—using insect repellent, nets, and protective clothing—and vaccination. The JE vaccine (Ixiaro) is recommended for those spending extended periods in rural parts of endemic countries or during peak transmission seasons.

Mild symptoms, if present, may include fever and headache. Severe disease can lead to inflammation of the brain (encephalitis), resulting in high fever, neck stiffness, seizures, coma, and permanent neurological damage. The case fatality rate in severe cases can be as high as 30%, with many survivors experiencing long-term complications.


A two-dose course of Ixiaro, given 28 days apart, offers protection for at least 1–2 years. A booster dose after 1 year extends immunity further. In some cases, accelerated schedules are available for last-minute travellers.


JE vaccination is recommended for travel to rural areas of countries such as India, Nepal, Cambodia, Thailand, Vietnam, Indonesia, China, and Japan, particularly if travelling during the rainy season or for extended stays.



Side effects are generally mild, such as soreness at the injection site, mild headache, or fatigue. Serious reactions are extremely rare. The vaccine provides strong protection and peace of mind when visiting high-risk areas.
